Wear and Tear-Arthritic or Muscular Pain

Hi all
I have met many people lately who are struggling with sore shoulders, unable to lift their arms and carry any weight.
What causes this painful condition that orthodox pain killers dont ease. It can be very debilitating and last for months even physio seems to be inneffective?
Is there a remedy to help this condition?
